Students who wish to participate in our Athletics program must complete all paperwork (Pre-participation Physical Evaluation, Parental Permission and Insurance Statement forms) listed in the documents section.

All completed paperwork must be uploaded to Aktivate and approved by our Athletic Directors prior to the student trying-out for any sport.
