Students who wish to participate in our Athletics program must complete all paperwork (Pre-participation Physical Evaluation, Parental Permission and Insurance Statement forms) listed in the documents section.

All completed paperwork must be presented to the Athletic Director, prior to the student trying-out for any sport.
